Discipline Resolution

In our ongoing quest to improve our Club, the CUSC has put in place a Director who will be responsible for all discipline and conflict resolutions within the Club.  Members who are in need of assistance or direction are to contact - Keith Currie

COMPLAINT PROCEDURE

Step 1Written complaint received. The complaint must be signed and dated.  The complaint must also be within the past 90 days and the person submitting the complaint must be willing to testify if necessary.

Step 2.  The complaint is forwarded to Discipline Chair and cc’d to the person in charge of the division in which the complaint was originated - House league, Rep teams or Referees

A copy of all complaints must go to the office to be logged.

Step 3. Decision of action to be taken.  Person handling complaint decides severity of the issue and how it should be handled.  If the matter is felt to be serious in nature, the President takes over deciding how to be handled.  The discipline committee will consist of 5 Board Members.  The secretary will be present to take notes and send communication.

A.     Discipline by review –The committee must consist only of persons that do not have any conflict of interest   in the matter at hand.
B.     Discipline by hearing – this is left for serious matters. Again, the committee must consist only of persons that do not have any conflict of interest in the matter at hand.

 Step 4 - Notification: A letter is sent to the individual that is being accused.  This letter must reach the accused at least 4 days prior to the review or hearing date.  The accused has the option to send a letter to state his/her case.  The accused also may request the review be changed to a hearing.  In this case there would be a monetary fee of $25 paid out by the accused as well as a form completed indicating his/her wishes.

 Step 5 – Discipline procedure.  The committee is assembled and either the review of evidence or the hearing takes place.  All evidence is documented and compared with OSA’s guidelines for discipline.

 Step 6 – Notification of decision.  A letter is sent to all parties involved indicating the findings of the discipline committee.  In the case of a verbal discussion a letter is also sent indicating the results of the discussion.  It is imperative that all matters are documented.  All letters are to be sent within 7 days of the decision date.

Step 7 - Appeal – the person that has been charged has the right to appeal.  In the appeal process a written appeal application is to be submitted to the office along with a monetary fee of $25.  If the appeal results in the change to a not guilty verdict and any penalties lifted the monetary value is returned.
